Each time you visit the hospital a medical record of your visit is made. A record contains your symptoms, test results, diagnoses, treatment and a plan of care. We refer to this record as your health or medical record. Your record is used for a variety of reasons primarily to plan your care and treatment. Your record may be computerized, stored electronically and on paper.
Release of Information
If you would like a copy of your medical record, print and complete the authorization to release medical records (.pdf) form and mail or fax it to the address below.
